I UNDERSTAND THAT I COULD BE HELD RESPONSIBLE FOR NOT FOLLOWING OPEN BURN REGULATIONS AS NOTED IN THE DOWNLOADABLE BROCHURE OR IF THE FIRE GETS OUT OF CONTROL CAUSING DAMAGE TO ANY PROPERTY.

  • Applicant must be the registered property owner or authorized agent in writing
